Our Census Form has three steps.
Step One: Fill out your school contact information below, then click on "Continue"
Step Two: Enter the employee information for your school.
Step Three: Review and edit your final entries and then submit them to CSI.

Note: You will be able to print a record of your final submission and also receive a summary confirmation via email. All information submitted will be kept strictly confidential.
