Insurance for Employees
$20,000 of life insurance.

Insurance for Covered Dependents
$4,000 for all enrolled spouses, and $2,000 of coverage on all covered
dependents.
Optional
Life Insurance
Employees and spouses may elect up to $250,000 of supplemental life
insurance.
Accidental
Death & Dismemberment
Schedule of coverage equal to life insurance amount. 24-hour coverage
provided.
Long
Term Disability
Monthly benefit equal to 2/3 of salary, subject to a maximum of $5,000
/ minimum of $150.
Adoption
Benefits
Provides up to $3,000 reimbursement for eligible expenses.
Dental
Plan - optional
100% coverage on diagnostic and preventive care.
Vision
Plan - optional
Annual eye exams, lenses and frames covered at 100%.
