Comprehensive Health Care
The following options are available effective September 1, 2004, with varying levels of coverage from 80-100%:

Trustmark PPO 90
Trustmark PPO 80
Trustmark PPO HSA 1

Prescriptions
PPO 90, PPO 80 and Plan C:
$15 generic co-pay / $30 preferred brand co-pay / $50 other brand co-pay

PPO HSA 1 and PPO HSA 2:
Covered at in-network deductible and 80% coinsurance

Life Insurance for Employees
$20,000 of life insurance.

Life Insurance for Covered Dependents
$4,000 for all enrolled spouses, and $2,000 of coverage on all covered dependents.

Optional Life Insurance
Employees and spouses may elect up to $250,000 of supplemental life insurance.

Accidental Death & Dismemberment
Schedule of coverage equal to life insurance amount. 24-hour coverage provided.

Long Term Disability
Monthly benefit equal to 2/3 of salary, subject to a maximum of $5,000 / minimum of $150.

Adoption Benefits
Provides up to $3,000 reimbursement for eligible expenses.

Dental Plan - optional
100% coverage on diagnostic and preventive care.

Vision Plan - optional
Annual eye exams, lenses and frames covered at 100%.
